Community Health Worker Specialists play crucial roles in the County of Sonoma, ensuring that individuals and families receive the support they need through education, counseling, and access to services. Working at the community level, these positions support improvements to programs and systems that support a well-integrated network of services and supports. The Community Health Worker Specialist positions that are currently open will work in the Public Health Division, Women, Infants, and Children (WIC) program or with the Tobacco-Nicotine Prevention and Enforcement Program (Tobacco Prevention Program). Depending on the program, these positions may provide quality and timely nutrition assessments, health goal setting, public outreach and education to the community and individuals, and support in a collaborative and dynamic environment, ensuring program participants receive the benefits and services they need. Additional responsibilities for these positions could include:
WIC Program Specific:
- Determining WIC program eligibility following WIC policies and procedures
- Providing both group and individual education and counseling on nutrition, breastfeeding, and other health-related topics
- Conducting comprehensive nutrition assessments, using available data (anthropometric, biochemical, environmental, and food/nutrition information), and creating personalized care plans
- Using motivational interviewing techniques to provide guidance and establish health goals
- Developing expertise in the program function and local, state, and federal laws
- Providing referrals to additional community resources as necessary, based on participant needs
- Working as part of a team to meet the demands of the WIC clinic, ensuring a high level of service and coordination across multiple sites
- Participating in ongoing training and certification as a WIC Nutrition Assistant (WNA) as mandated by CDPH WIC
Tobacco Prevention Program Specific:
- Engaging and educating diverse audiences through different media, including social media and in-person presentations
- Experience with, or an understanding of, how local ordinances are passed and implemented, including how the public can be engaged in this process
- Possession of presentation/public speaking skills
- Ability to develop expertise in local, state, and federal tobacco laws
- A Community Health Worker Certificate is desirable, but not required

Program Information WIC (Women, Infants, and Children) is a nutrition program designed to protect and improve the health of low-income pregnant, breastfeeding, and postpartum women, as well as infants and children up to the age of five. WIC provides healthy foods, nutrition education and counseling, breastfeeding support, and referrals to other services in the community. WIC loans electric breast pumps to participants and has a hospital exchange program with local hospitals. Lactation services are also provided by an International Board Certified Lactation Consultant (IBCLC). The work schedule for the position that is currently open in the WIC program is Monday to Friday, from 8 am to 5 pm, and from 10 am to 7 pm once or twice a week. You will rotate across all four WIC sites: Santa Rosa, Sonoma, Petaluma, and Guerneville.
Tobacco Prevention Program: The Community Health Worker Specialists in the Tobacco-Nicotine Prevention and Enforcement Program will perform a variety of tasks to advance the scope of work for the California Tobacco Prevention Program tobacco retail compliance program and the related compliance activities. The primary work involves public outreach and making connections with community organizations and individuals. This requires having experience and an understanding of how to engage various audiences through media, especially social media and other activities. The development of effective educational materials, representing the program at community events and meetings, and developing professional flyers for distribution are key components of this work. This work also involves developing expertise in local, state, and federal tobacco policy, understanding the health effects of tobacco use, the variety of tobacco products on the market, their use patterns, and prevalence in consideration of a variety of social and demographic strata. The position will assist in providing backbone support to the local tobacco coalition and will attend a variety of meetings where short updates or presentations are made. The position also leads and supports training on a variety of topics related to passing and implementing tobacco policies, and on the skills needed to engage with partners, organizations, leaders, and policymakers.
